Stored procedure like Microsoft Access cross tab query

Expert Kevin Kline discusses cross tab queries in this tip.

How do I create a stored procedure that collects data like the Microsoft Access cross tab query?
Cross tabs are not easy, by default, in SQL Server. Fortunately, my colleague and friend Andrew Zanevsky solved this challenge with a stored procedure called sp_crosstab way back in the June 2000 issue of SQL Server Professional.
